Create custom statuses

Ability to create custom statuses.

Currently they are fixed as:
1. Rehab/Unavailable

2. Modified

3. Assess

4. Full

5.Away

Give the user the ability to create and edit the statuses an athlete could be assigned.

    Had a call with the client, they wanted this to essentially help assess more precisely how far an athlete is off full/RTP

    Ex:
    Rehab
    Very Restricted
    Modified
    Slightly Mod
    Full

    Just a note - we will still need status categories, e.g. "Available", "Unavailable", "Modified", "Other", for the Player Status Dashboard widget to work correctly. Hopefully those categories cover the necessary cases.
